Step 1
Submit a Student Application with copies of the following documents:
Please note that all transfer students are admitted on a probationary status for one school semester.
Step 2
Once the student is admitted, complete the Registration and Tuition Agreement and setup your payment plan with FACTS Tuition Management Program. FACTS is easy to enroll in, just click on https://online.factsmgt.com/signin/48G7G. Tuition Assistance is available for all families that qualify.

- ____ Immunization records up to date.
- ____ Includes Hepatitis B vaccine and Varicella immunization or immunity
- ____ Complies with Tdap booster shot & 2nd MMR (required for 7th grade)
- ____ Birth Certificate
- ____ Baptismal Certificate
- ____ Physicians checkup (Kindergarten and First Grade only - green form)
- ____ Copy of last report card at previous school (First - 8th grade)
- ____ Standardized test scores from last school year (2nd-8th grade)
- ____ Make an appointment for an assessment and interview after the application is complete.
